How to Register
In order to reserve your place, you will need to complete and submit a registration form and pay in full.
The registration form contains important Vaccination, Training Agreement and Liability Release information. Please complete and sign either by using our online form or by downloading a pdf to print (you can scan and email, fax, or send via snail mail with check.
In addition, we need a proof of up-to-date vaccinations (a hard copy or pdf of letter or detailed receipt) from your vet that we keep on file.
There are two ways to register: online with a credit card payment or download a form and send in (or drop-off) with a check.
Register online
Registering online is a one-step process. You can fill out the form, upload a pdf proof of vaccinations, and make a payment ($305 ).
Register by sending a check
If you want to pay by check, please download and complete a registration form, attach proof of vaccinations, and send with payment to:

Important Notes
A spot in class will be saved for you only after you have completed payment. Once payment is received for class it is NON-REFUNDABLE.
Please plan to attend all class dates as each class builds on the last. There are no make ups available for missed classes.
PLEASE NOTE: All human participants must be 18 years of age or older to attend Punky Pups classes with their pup for safety reasons.
